Required Information

     LIST OF REQUIRED INFORMATION IN ORDER TO MAKE A BOOKING
1. Owners name, address, telephone and email.
2. Local contact who could collect the dog(s) in an emergency at the Kennels.
Name, Address, Telephone and email is also required to comply with the Stratford Council licensing Rules.
3. Any other names and telephone numbers of people you would wish to speak to
the vet or for any other reason. Addresses are not required for these.
4. The name of the dogs Veterinary Practice, Address & Telephone No.
5. Name of dogs Insurer if insured, not compulsory.
6. Up to date vaccination certificate, kennel cough advised, (more details on the consent form) Puppies have to have completed their full course of primary vaccinations,
at least two weeks prior to boarding.
7. Dogs microchip no.
8. Details of the dog food and how many times fed.
9. Details about your dog, spayed, breed, age, etc. (all put onto a booking form)
10. Details of flea and worm treatments, name of product, date given and the normal
pattern eg every month 1.7.2024 Nexgard Specta
11.A consent form has to be signed before a booking is confirmed and all of the above
information provided.
